Tax Check List

Name, social security number, date of birth, for taxpayer, spouse, and any dependents.

All W-2 and 1099 forms received.

Receipts for all medical expenses including health insurance premiums paid out of pocket (do not include amounts reimbursed by insurance company). Mileage for medical purposes.

Property taxes and interest paid on mortgage; any points paid for mortgage loans or refinance; closing papers on sale or purchase of home.

All charitable contributions made during the year including items donated to organizations such as Good Will, or the Salvation Army.

DMV registration fees. Taxes on personal property such as boats, RV’s, etc.

Any work related expenses not reimbursed by employer, including mileage and auto expenses.

All stock and investment records - purchases and sales.

Gambling winnings and losses.

Tax preparation fees.

Any other expenses you feel may be pertinent.
